Our goal is to cover everything you need with a full and efficient assessment, but without waiting to see the doctor.

The accuracy of your claims, proper reminder cards, etc., are all dependent on the account information we obtain from you. We only collect the information that is necessary to conduct our business and to provide you with comprehensive quality health care. The information in your file will only be shared with the individuals or businesses that you have given us written permission to share it with, such as your insurance company, another physician, etc. We maintain strict physical, electronic and procedural safeguards to protect your personal information. We offer patient forms online so you can complete them in the convenience of your own home or office. Fax us your printed and completed forms or you may bring them during your visit.
All New Patients Required:
- HIPAA Compliance Form– Use this form after you have read the HIPAA compliance information provided. Please fill out one form for each patient.
- Then choose among the appropriate cateogories below, adult or child:
- Child Registration and Health History
- Under 18 Consent Form– Use this form if your child is coming to our practice without his/her legal guardian or parent.
